Good Faith Estimate

Your rights under the No Surprises Act.

You have the right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your care will cost. Under the law, health care providers must give patients who don't have insurance, or who are not using insurance, an estimate of the expected charges for medical services.

What that means here

AMW Physical Therapy is an out-of-network, cash-pay practice. You can ask for a Good Faith Estimate before you schedule, and we will provide one in writing at least one business day before your visit for services scheduled at least three business days in advance.

If your bill is higher

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Keep a copy of your estimate.

Current rates

Current rates for evaluations, follow-ups, concierge and virtual visits are listed on our booking page, so you can see the cost of every visit type before you book. No guesswork, no surprise bills.
